Job Description of Staff Nurse as per the Government of Nepal

Tasks, Duties, and Responsibilities to be Performed:
Tasks must be performed in accordance with the Nepal Health Service Act and Regulations under the supervision and authority of the Hospital Chief and Chief Nurse, following the Code of Ethics prepared by the Nepal Nursing Council.
Regarding Service Delivery
1. Inpatient Services:
- Obtain information about the condition of patients admitted in the assigned working area through handover/takeover, and understand patient status as needed.
- Examine admitted patients as per the nursing process and provide quality nursing care.
- Keep records of patients' critical health conditions and collect complaints for immediate management as required.
- If service delivery is needed in intensive care units such as ICU, CCU, NICU, OCMC, services shall be provided personally or based on the knowledge, skills, and competencies of subordinate nursing staff.
- Assess the patient's condition and the quality of nursing and midwifery services provided, as per the nursing process, and take corrective actions.
- Evaluate patients' progress based on services provided and inform the relevant nursing officer and physician as needed for management.
- Delegate nursing tasks to subordinates based on their competency.
- Make arrangements/support for continuous 24-hour patient care service in the hospital.
- Implement appropriate infection prevention and waste management measures to protect patients, visitors, and healthcare workers from hospital-acquired infections.
- Assist in maintaining cleanliness of the hospital environment.
- Participate in Continuing Professional Development (CPD) programs and implement new, timely, and updated nursing practices for quality service delivery.
 
2. Maternity Services:
- When involved in maternity care, provide respectful maternity care, ensure no woman feels any form of obstetric violence, and take corrective measures for ensuring violence-free service delivery.
- Provide necessary counseling individually and in groups to pregnant women and their families regarding nutrition, hygiene, immunization, and birth preparedness.
- Assist pregnant women and their families in planning for delivery.
- Conduct antenatal checkups and provide counseling to pregnant women and their husbands for normal delivery preparedness.
- Fill out partograph for laboring women, immediately manage any abnormalities found, and report to higher authorities.
- Provide continuous care to mothers and newborns during pregnancy, childbirth, postpartum, and beyond (Continuum of Care).
- Assist in promptly managing any complications found in mothers and newborns.
- Provide normal delivery services respecting the woman's choice of birthing position, and offer PPIUCD (Postpartum Intrauterine Contraceptive Device) upon consent after delivery.
- Provide postpartum and neonatal care as needed, such as perineal care, breast care, breastfeeding support, etc.
- Facilitate breathing for newborns (Helping Babies Breathe) to assist with normal respiration.
- Detect and address common postpartum problems in mothers and newborns, identify complex problems timely, inform physicians, or provide necessary nursing and health services, including referrals.
- If the health facility is accredited and the nurse is trained, provide standard abortion services according to regulations.
- In the case of incomplete abortion (PAC), trained nurses shall provide abortion services or refer the case to an appropriate health facility if the service is unavailable.
- Provide counseling to family planning clients and couples and distribute family planning commodities.
 
3. Intensive Care Unit (ICU) Services:
- Obtain information about the condition of admitted patients through handover/takeover and understand patient status as needed.
- Examine admitted patients and prepare a nursing care plan for each patient according to the nursing process.
- Identify patient needs and provide quality nursing care according to standard treatment protocols.
- Discuss patient status with the healthcare team and assist in providing other healthcare services following standard treatment protocols.
- Ensure the operational status of ventilators, monitors, and other equipment, and after use, disinfect and store them appropriately following infection prevention and control measures.
- Maintain records of malfunctioning equipment and initiate repair processes.
- Keep complete records of patients' health conditions and collect complaints for immediate management.
- Assess the quality of nursing services provided as per the nursing process and take corrective actions if necessary.
- Evaluate the patient's progress based on nursing services provided and inform the nursing officer and physician as necessary for management.
- Implement appropriate infection prevention and waste management measures to protect patients, visitors, and healthcare workers from hospital-acquired infections.
- Provide necessary counseling to patients and their relatives.
4. Dialysis Services:
- Examine and record the patient's condition before, during, and after dialysis.
- Examine patients according to the nursing process and prepare a nursing care plan for each patient.
- Provide hemodialysis and peritoneal dialysis services using appropriate methods.
- Evaluate the dialysis services provided and take corrective actions.
- In case of any abnormalities during service provision, immediately inform the nursing officer and physician for necessary management.
- Ensure the operational status of dialysis machines and tools in cooperation with dialysis technicians and store them properly after use.
- Provide counseling to patients and their relatives about dialysis techniques, alternative treatments, kidney disease management, home care post-dialysis, diet, and physical exercises.
5. Pediatric Ward:
- Examine admitted children according to the nursing process, prepare a nursing care plan for each patient, and provide services accordingly.
- Assess the quality of nursing services being provided as per the nursing process and take corrective actions as necessary.
- During service delivery, in case of abnormal conditions, immediately inform the Nursing Officer and Physician for necessary management.
6. Operation Theatre:
- Proper management, storage, use, and recording of surgical instruments used during operations.
- Sterilize and maintain readiness of surgical instruments used during operations.
- Arrange for proper cleaning of the operation room and assist in sterilization.
- Before surgery, verify that all patient files, investigation reports, and required blood products are available.
- Prepare patients for surgery.
- Ensure and respect the confidentiality and privacy of patients' personal information according to their rights and needs.
- The scrub nurse must prepare the operation table and surgical materials on time for the surgery.
- Provide quality nursing care before, during, and after surgery, and maintain records accordingly.
- Use surgical materials and instruments properly and perform counts before, during, and after surgery.
- Properly dispose of single-use materials after surgery, and send reusable items for decontamination and sterilization.
- Maintain complete health records of patients.
7. Post-Operation Ward:
- Continuously monitor and record patients' vital signs until they regain full consciousness after surgery.
- Assess patients' pain and conduct necessary clinical examinations and investigations.
- Administer appropriate medications and techniques for pain management.
- Upon confirmation of full recovery by the anesthesiologist, transfer patients to an appropriate ward or discharge them home.
8. Emergency Department:
- Quickly assess patients' health conditions, identify the main problems, and provide prompt and quality nursing care while maintaining full patient health records.
- Work in coordination with all health staff to provide quality emergency healthcare services.
- Conduct diagnostic tests as necessary and deliver swift emergency care.
- Keep life-saving medications and resuscitation trolleys ready, and administer life-saving medications and resuscitation as needed.
- Prepare and maintain a disaster preparedness plan for hospital readiness in case of natural disasters.
- Immediately inform the Nursing Officer and Physician for necessary management when abnormal conditions are encountered during service delivery.
9. Outpatient Department:
- Identify diagnostic and therapeutic services required for patients visiting the hospital and arrange to provide services promptly and effectively.
- Arrange services related to adolescent health, family planning, antenatal care, routine immunization, reproductive health, communicable diseases, geriatric care, and child health.
- Ensure the availability of safe motherhood services, newborn services, education, and counseling as needed for postnatal mothers and newborns.
- Provide special priority and immediate services to referred and emergency patients.
- Provide safe abortion services.
10. Referral:
- Facilitate and develop a two-way referral system between the local health institutions and other health facilities for maternity and gynecological services.
Administrative-Related Tasks
1. Planning:
- Assist in preparing duty rosters for nursing and support staff at the workplace.
- Collaborate with concerned staff to plan programs to enhance the quality and effectiveness of services like Safe Motherhood, Family Planning, Safe Abortion, Adolescent-Friendly Services, and Infection Prevention by analyzing available indicators.
- Analyze past achievements related to services under one's responsibility and formulate branch or unit plans.
- Propose plans and programs to the Nursing/Office Chief for staff skill and knowledge enhancement through various trainings and programs.
- Prepare records and documents necessary to propose plans and programs for mobilizing local resources.
2. Natural Disaster and Epidemic Response:
- Be actively involved in preparedness and response to control natural disasters and epidemics, and support other related activities.
- Provide or facilitate emergency services in case of unpleasant events (such as accidents, snake bites, burns, etc.).
3. Monitoring, Evaluation, and Supervision:
- Supervise subordinate staff and ensure that clients receive respectful services, including dignified maternity care.
- Provide training to trainees and monitor their performance.
- Regularly conduct client satisfaction surveys and initiate improvements based on the findings.
4. Health Education and Counselling:
- Provide necessary health education and counselling to patients and caregivers for health promotion.
- Inform patients and caregivers about hospital policies and provide health education and counselling as needed.
- Deliver health education related to maternal and child health, nutrition, diarrhea, and respiratory diseases, and monitor the effectiveness of health education.
- Support the establishment and operation of health education corners.
- Ensure mothers are breastfeeding and provide education about the importance of breastfeeding.
5. Medication Administration:
- Administer medicines and injections according to physicians' written prescriptions and the established standard treatment protocols under the Nepal Nursing Council's Code of Ethics and Scope of Practice.
- Bring necessary medicines from the store and maintain them properly.
6. Supply Management:
- Request, maintain, and properly manage equipment and materials under one’s responsibility.
- Ensure the arrangement of necessary medicines and other supplies.
7. Record Keeping:
- Maintain inventory records of medicines and materials needed for emergency treatment.
- Maintain nursing records about patients' conditions and report accordingly.
Other Responsibilities:
- Act as the Nursing Chief of designated wards as assigned.
- Conduct fair evaluations of subordinate staff and provide feedback to improve quality.
- Participate in orientation workshops and celebrations of national health days related to various health programs.
- Perform other tasks according to acquired training and skills.
